Notification Form

Ohio law requires you to notify your local school district (see below for a list) of your intent to homeschool. A form is provided from the state of Ohio to accomplish this purpose. Click here for the form (PDF).

Members of the Home School Legal Defense Association (HSLDA) should use the HSLDA form, which can be obtained from the HSLDA website. Also, the notification must include information on the annual achievement tests for your children. HSLDA has another form, called Ohio Test Results Reporting. This form eliminates information not required by law, therefore we highly recommend the use of this form for HSLDA members.

Please call before your initial notification to ensure that you are sending your form to the correct office.
